These Terms and Conditions for delegates are valid for every person registered to attend the 2nd International HIV/Viral Hepatitis Co-infection Meeting on 17 & 18 July 2015 in Vancouver, Canada.
Only fully completed registration forms will be accepted. The registration will only be confirmed upon receipt of payment in full. Once the maximum capacity is reached, the registration system will close and the symposium organizers reserve the right to refuse additional registrations.
A hard copy of the symposium abstract book is only guaranteed if the delegate has indicated in the registration form that they would like to receive one. The 2nd International HIV/Viral Hepatitis Co-infection Meeting has reduced the number of printed symposium materials in order to save paper in an effort to go green. The registration fee guarantees on-site symposium materials including the programme book, coffee breaks, and lunches each day and a networking reception.
A registration confirmation will be sent by email after the 2nd International HIV/Viral Hepatitis Co-infection Meeting Organizing Committee has received the fully completed registration form, the related payment and any required supporting documents. Delegates may be requested to present the registration confirmation onsite at the registration counter as proof of their registration and payment.
Methods of payment
The registration fee must accompany all symposium registrations. Payments should be made in advance and by using a credit card only.
Registration name transfer
If a delegate would like to request to transfer their registration to a colleague, they must make a request in writing to the 2nd International HIV/Viral Hepatitis Co-infection Meeting Organizing Committee by email no later than June 15, 2015. After this date, no name changes will be carried out. A new registration form for the substitute delegate should NOT be submitted.
Please note that registration transfers for those delegates who have registered at the discounted rate of $65 Cdn can only be transferred to an IAS 2015 registered delegate. The substitute delegates IAS 2015 registration ID will be required to complete the transfer.
Notification of cancellation must be made in writing and sent to the 2nd International HIV/Viral Hepatitis Co-infection Meeting Organizing Committee by email.
Cancellations received on or before June 15 2015 will be refunded, less an administration fee of $20.00. Cancellations after June 15, 2015 are not refundable. Exceptions are at the discretion of the Organizing Committee.
In the case of overpayment or double payment, refund requests must be made in writing and sent to the 2nd International HIV/Viral Hepatitis Co-infection Meeting Organizing Committee, by email, no later than 17 July 2015. A handling fee of Cdn 20 will be charged.
Refunds will be made in the same form as the original transaction.
Credit will not be given for unattended events or early termination of attendance. Refunds will also not be made to delegates who register to the 2nd International HIV/Viral Hepatitis Co-infection Meeting as a non‐conference delegate to IAS 2015, but later register for the IAS 2015 Conference. The IAS 2015 registration ID number must be provided at the time of registering to the 2nd International HIV/Viral Hepatitis Co-infection Meeting in order to benefit from the discounted rate.
Cancellation of the Symposium
In the event that the symposium cannot be held or is postponed due to events beyond the control of the symposium organizers (force majeure) or due to events which are not attributable to wrongful intent or gross negligence of the symposium organizers, the symposium organizers cannot be held liable by delegate for any damage, costs, or losses incurred such as transportation costs, accommodation costs, costs for additional orders, financial losses, etc.
Under these circumstances, the symposium organizers reserve the right to either retain the entire registration fee and to use it for a future symposium, or to reimburse the delegate after deducting costs already incurred for the organization of the symposium and which could not be recovered from third parties.
Modification of the programme
The symposium organizers reserve the right to modify the programme, which is published as an indication only.
The symposium name badge must be worn at all times during the symposium. Access to the symposium venue will not be granted without the symposium name badge (IAS 2015 conference badges are not valid).
Letter of invitation
Individuals requiring an official Letter of Invitation from the symposium organizers can request one through the online registration form or by contacting the 2nd International HIV/Viral Hepatitis Co-infection Meeting Organizing Committee. To receive a Letter of Invitation, delegates must first register to the symposium, pay in full and submit any required supporting documentation (if applicable).
The Letter of Invitation does not financially obligate the conference organizers in any way, nor does it guarantee an entry visa to Canada. All expenses incurred in relation to the symposium are the sole responsibility of the delegate.
It is the sole responsibility of the delegate to take care of his/her visa requirements. Delegates who require an entry visa must allow sufficient time for the application procedure. Delegates should contact the nearest Canadian embassy or consulate to determine the appropriate timing of their visa applications.
All visa applications should be submitted to the authorities one month before travelling to Canada at the latest (i.e., no later than 15 June 2015).
Delegate registration details may be shared with the Canadian immigration authorities to assist in the immigration process. However, the Symposium organizers will not directly contact embassies and consulates on behalf of the delegates.
The registration fee minus a handling fee of Cdn 20 will be refunded after the symposium if the visa was applied for in time (i.e., no later than 15 June 2015) and proof is shown that a visa could not be granted even though all requested documents were submitted. Refund requests must be made in writing and sent to the 2nd International HIV/Viral Hepatitis Co-infection Meeting Organizing Committee, by email, no later than 18 July 2015. No refund requests will be accepted after this date.
Data protection and sharing of contact details
The 2nd International HIV/Viral Hepatitis Co-infection Meeting Organizing Committee will collect and store all personal data for the preparation and execution of the 2nd International HIV/Viral Hepatitis Co-infection Meeting and future symposia. By agreeing to the Terms and Conditions, delegates understand that the data they have provided may be used (saved, stored, processed, transmitted, and deleted) to process their application. In addition, if the delegate gives permission the 2nd International HIV/Viral Hepatitis Co-infection Meeting Organizing Committee may share contact details of delegates with the symposium sponsors and other third parties that may use these details to inform delegates of other activities surrounding the 2nd International HIV/Viral Hepatitis Co-infection Meeting and the IAS 2015 Conference or other communications which may be of interest.
The symposium organizers shall be held liable in the framework of a duty of care as a respectable business according to statutory provisions. The liability of the symposium organizers – for whatever legal reason – shall be limited to intent and gross negligence. The liability of commissioned service providers shall remain unaffected by this. The delegate shall take part in the symposium at his/her own risk. Oral agreements shall not be binding if these have not been confirmed in writing by the 2nd International HIV/Viral Hepatitis Co-infection Meeting Organizing Committee.

You are responsible for booking your own accommodation. If you are attending IAS 2015 we suggest you book your accommodation through their registration system.
In registering for this event you will be paying the Providence Health Care Research Institute. Online registration payments can be made using Visa or Mastercard only. Charges will appear on your statement as coming from the CTN.

Registration fees do not include personal insurance of any kind. The 2015 International HIV/Viral Hepatitis Co-Infection Meeting Organizing Committee do not take responsibility for any delegate.
